Barrier Failure Analysis training

Our Barrier Failure Analysis training consists of a two-day group training. The training can be delivered in a live session or as an online course based on your preference. In the training, the following subjects are covered: incident causation theory, information gathering, interviewing techniques, information structuring, BFA analysis (with the tool), reporting (with the tool), formulating safety recommendations, effective implementation of safety measures, evaluating measures, and learning from incidents. The training can further be tailored to include:

  • A "common language" between BFA and Tripod Beta, to assist in conducting meta-analyses if necessary.
  • The use of examples consistent with your daily practice.
  • The development of a specific working method manual, consistent with practical and relevant situations.

About Barrier Failure Analysis

Barrier Failure Analysis (BFA) is a relatively simple and format-free method, based on the same barrier-based thinking as Tripod Beta. Because it is based on barrier thinking, BFA offers better guidance than 5Why to identify underlying causes, and it allows for multiple causation paths. Furthermore, BFA can quite easily be connected to existing Bowtie risk analyses. Being relatively format-free, BFA offers a chance to develop a tailor-made format, and even develop a tool with guidance and for reporting that is specific for your sector.
