Tripod Beta is an incident analysis method owned and maintained by Energy Institute (EI). Practitioners are accredited by EI according to bronze, silver and gold status; gold status is appropriate for incident investigators who regularly conduct Tripod Beta analyses.

Tripod Beta - Road to Gold

Human Safety Academy provides training (by an accredited trainer), coaching and support throughout the process to Tripod Beta Gold status. An application manual and handy reference guide are provided for each participant. Furthermore, templates of the building blocks are provided; use of specific software is optional.
The road to Tripod Gold consists in total of four steps, visualized in the figure below, together with the duration of each step. TThe entire training, its contents and the duration can be tailor-made based on the experience and proficiency level of your investigators.

About Tripod Beta

Tripod Beta was developed to aid in the analysis of process disruptions (incidents) so that incident investigators could comprehend the effects on people of the operational environment (where the risks are) in which the incident occurred. With this knowledge, it is possible to pinpoint the organisational flaws that contributed to this environment and make improvements.

The Swiss Cheese model and the Human Behaviour model are the foundations of the Tripod Beta approach. Both theories are founded on science and have their roots in the academic work of professors W.A. Wagenaar and J.T. Reason from the universities of Leiden and Manchester, respectively. The method focuses on 3 questions:
